To comply with Maryland regulations, in order for your child to be able to attend our summer camp, please, fill out the following forms:
Form SC1- Camper Health Form (one per camper )
Form SC1-bis- Immunization form (one per camper - ONLY for campers attending schools outside of Maryland- please print and fill out - if you prefer you can replace this form by an up-to-date copy of vaccination records of your child)
Form SC2 - Notice to send non-perishable food (one per camper )
Form SC3 - Sunscreen and insect repellent authorization form (one per camper )
Form SC4 - Camp participant statement of understanding (one per family )
Form SC5 - Camp participant code of conduct (one per camper - please print, sign (by parent AND camper) and send or bring back at the latest first day of camp )
Form SC6 - Release of liability (OPTIONAL - only for campers who are at least 13 years old )
Additional forms for campers who take the bus:
Form SC8 - Bus Authorization
Form SC9 - Bus Code of Conduct (one per student - paper)
