This page provides access to all sports forms.
Forms 1 to 3 are mandatory for all students and have to be re-submitted for each school year.
(One time per school year, whether the student participates only in the classes or only the (sports) immersion programs, or on both.)
Form 1 - Health Form (one per student - online only - please submit before first day of attendance)
Form 2 - Code of Conduct (one per student - paper - please print at home, sign, and hand to teacher on first day of attenance)
Form 3 - Waiver and Release Agreement (one per family - online only - please submit before first day of attendance)

Form 4 - Immunization Form (one per student - paper- to be filled out ONLY for students who do not attend school in Maryland)
Form 5 - Sports Program Waiver (one per student - online only - to be filled out ONLY for students who participate in one of our sports programs- please submit before first day of attendance of sports program)
