This page provides access to all school forms.
Please read - My French Classes rules and regulations
Forms 1 to 3 are mandatory for all students and have to be re-submitted for each school year.
Form 1 - Health Form (one per student - online only - please submit before first day of attendance)
IMPORTANT : Students with their weekday school in Maryland do not need to bring an immunization record signed by their pediatrician.
All other students need to bring one but can bring a simple copy of the pediatrician-signed immunization record submitted to their respective schools in DC, VA, or other states. (It is not necessary to ask your pediatrican to sign another original.)
Form 2 - Code of Conduct (one per student - paper - please print at home, sign, and hand to teacher on first day of attenance)
Form 3 - Waiver and Release Agreement (one per family - online only - please submit before first day of attendance)
Update Forms
Form A - Family Information Update and Preferences (online only, to complete your family's registration process and/or update your information during the year)
(Inludes the possibility to register the email address of a second person.)
Form B - French Level Information (online only, FOR NEW STUDENTS)
Form C - If the student is in 6th grade or above: Student Email Submission (to receive the weekly teacher email directly)
Forms 4 to 10
You may be asked to return one of the forms below in relation to certain programs or circumstances.
Form 4 - Immunization Form (one per student - paper- to be filled out ONLY for students who do not attend school in Maryland)
Form 5 - Walk Home Release (one per student - paper - optional and only for students 6th Grade / 6e and up)
Form 6 - Sports Program Waiver (one per student - online only - to be filled out ONLY for students who participate in one of our sports programs- please submit before first day of attendance of sports program)

Form 7 - Bus Authorization (one per student - online only -please submit before first day of riding the bus)
